Please contact me regarding fees. I am a Blue Cross preferred provider. For other insurance, I will provide you a receipt to file with your insurance company for reimbursement. Payment is requested at the time of service, payable by cash or check (sorry, no credit cards accepted at this time).
I do not provide services to anyone living outside of the state of Alaska (United States).
